Registration with $250.00 deposit is due in our office at the time of sign-up.   Payment of $1597.50 is due in our office by 3pm, Thursday, July 31, 2008. Final payment is due Thursday, Sept 18^th, 2008.

CANCELLATION POLICY:

You may cancel your participation in the tour up until July 31, 2008 without penalty or loss.  

Cancellation Fees:  Aug 1st-Sept 18^th - 50% of the total Tour Price

                                After Sept 18^th  - 100% of the Total Tour Price.  

This is because all payments will be disbursed to the Airlines and Tour Company.   Once tickets are issued by the airlines they are non-refundable and non-changeable.  Travel insurance is advisable.

This form must be completely filled out by each individual tour participant and returned to MasterMedia with your deposit.  To avoid mistakes please PRINT or TYPE all answers.  You will need a CURRENT VALID PASSPORT to travel outside the USA.  Important Note: If your Passport in not valid for 4-6 months PAST the return date of your tour – please apply for a new one immediately.  This is a new requirement since 9-11.  If you do not yet have your Passport information go ahead and fill out the rest of this form then call us when your Passport info becomes available.  Citizens of countries other than the USA may need an entry VISA.  It is the responsibility of the passenger to obtain the necessary travel documents required to travel.
